Reviewed release commit b282b0b367bc5f3c8241b47c3eb39f24c68492d7 and its state event.
+2. Actual fresh reviewer success: this QA process started at 2026-10-14T18:56:00+00:00, after release and the executor submission commit. It successfully issued and consumed shell/Python tools, including task CLI reads, independent readiness assertions and validation. Its startup session is 01a13bc5-7ca6-7903-a55d-52bccd0f6ee3. Failed QA, recovery diagnosis executor, post-release resolver and current QA have four distinct session ids; all headers show Codex v0.162.1, gpt-6.1-sol, provider openai. runner.local.json has no role overrides or resume argument; runner.py reads the persisted UTF-8 prompt through stdin and assigns fresh process metadata. No new harness/model process was launched by this reviewer.

Browser/server/application interoperability, S00-012 implementation and S00-002 stack acceptance are outside this review. No credentials or provider/configuration changes were needed. No acceptance defect or review blocker found. Coordinator must commit the review transition before launching the next fresh session; reviewer stops after approve and does not mutate Git.
