wt 944f399c95 S00-016: Approve recovered reviewer availability after fresh QA tool checks
Changes: Accept only S00-016 readiness and add independent QA evidence after verifying coordinator release, executor patch and fresh reviewer tool use.
Validation: Read-only readiness assertions passed; tasks.py validate passed for 281 tasks at revision 30; git diff --check passed. Verified distinct same-model sessions and preserved parent acceptance boundaries. Future provider capacity and parent implementation reviews remain separate.

Task title: Restore reviewer model availability after provider capacity failure

S00-016 independent QA review

Reviewer: auto-qa-309d45d4fd4b (qa). Executor: auto-ci-cd-351a0cbbabef (ci-cd). Date: 2026-10-15. Decision: approve the observed recovered reviewer execution path.

Input: assigned card/context packet, startup/workflow instructions, all required task inputs, blocker records, executor evidence/handoff/message, accepted ADR 0000 and direct dependency executor/review evidence. Output: this independent readiness decision. No source or harness configuration changes.

Acceptance findings

  1. Explicit readiness: coordinator-capacity-b12fb7abdd2d released S00-016 at revision 28, 2026-10-14T18:54:03+00:00. The persisted reason authorizes fresh verification of recovered availability through the existing shared command and preserves independent parent acceptance. Reviewed release commit b282b0b367 and its state event.
  2. Actual fresh reviewer success: this QA process started at 2026-10-14T18:56:00+00:00, after release and the executor submission commit. It successfully issued and consumed shell/Python tools, including task CLI reads, independent readiness assertions and validation. Its startup session is 01a13bc5-7ca6-7903-a55d-52bccd0f6ee3. Failed QA, recovery diagnosis executor, post-release resolver and current QA have four distinct session ids; all headers show Codex v0.162.1, gpt-6.1-sol, provider openai. runner.local.json has no role overrides or resume argument; runner.py reads the persisted UTF-8 prompt through stdin and assigns fresh process metadata. No new harness/model process was launched by this reviewer.
  3. Parent acceptance preserved: before this decision, S00-012, S00-015 and S00-002 remain blocked; direct prerequisites S00-001/011/013/014 are done. This review accepts only S00-016. The CLI may return S00-015 to its saved queue state on resolver approval; it does not constitute S00-012 or S00-002 acceptance. Subsequent work/reviews require separate fresh coordinator sessions.

The original failed QA run auto-qa-98b05bd56c25 has returncode 1, stopped_after_transition=false and log text HTTP 503 Service Unavailable / Selected model is at capacity. The post-release executor completed with status=finished, result.status=review, stopped_after_transition=true, returncode=-15 and no scope violations. This is the documented runner stop after submit, not a provider failure. Its recorded commit is 83404f9c8a. Reviewed its full seven-file patch and message: only evidence, handoff, commit message, CLI state and generated matrices changed. Executor message accurately describes actual observations/checks and limits. Initial working tree was clean.

Independent commands and actual results

  • python3 scripts/tasks.py resume --agent auto-qa-309d45d4fd4b: exit 0, empty owned-task list as expected for reviewer; no claim.
  • python3 scripts/tasks.py show S00-016 and packet S00-016 --agent auto-qa-309d45d4fd4b: exit 0 each; assigned review and bounded acceptance inspected.
  • Required artifact/card/handoff/evidence/message reads: present and inspected. An initial optional guessed ADR filename read returned exit 1; actual accepted ADR docs/decisions/0000-scope.md was subsequently read successfully. No required artifact is missing.
  • git log --all --format='%h %s' --grep='^S00-016:', git show 83404f9, git show b282b0b --format=fuller -- state/tasks.json and git status --short: exit 0; full executor patch/message and release reviewed, initial tree clean.
  • Read-only Python readiness probe over local config, state/events, four run headers/records and named assignment variables: final exit 0. Asserted explicit release, common command/no resume, four distinct same-model sessions, original failure, committed successful executor transition, fresh QA assignment/tool use, done prerequisites and blocked/unaccepted parents. Output: PASS: explicit release; shared command; four distinct same-model sessions; real fresh QA tool use; committed executor; parents remain unaccepted.
  • First draft probe exited 1 because it incorrectly expected run.status=review rather than finished with result.status=review. Corrected the assertion against the actual runner record and reran successfully; no product/source change or unresolved failure.
  • python3 scripts/tasks.py validate: exit 0; 281 tasks, 22 stages, 221 requirements, 211 allocated Web requirements, revision 30.
  • git diff --check: exit 0 before report; report/message content checked before approve.

Limits and handoff

This is observed provider recovery and real fresh QA tool availability, not a guarantee of future capacity. The current QA run is still running; its final returncode/commit will be recorded by the coordinator after the terminal transition. No new tests were added and no source test suite was rerun for this evidence-only task. Browser/server/application interoperability, S00-012 implementation and S00-002 stack acceptance are outside this review. No credentials or provider/configuration changes were needed. No acceptance defect or review blocker found. Coordinator must commit the review transition before launching the next fresh session; reviewer stops after approve and does not mutate Git.
